Osmidrosis (Underarm Odor)

What is Osmidrosis (Underarm Odor)?

Osmidrosis, sometimes called bromhidrosis, occurs when secretions from the apocrine glands in the underarms are broken down by skin bacteria, producing a strong odor. Apocrine glands become active at puberty, and genetics play a significant role. People with wet, sticky earwax are more likely to have active apocrine glands. Excess sweating, heat, friction and hair in the underarm area can all intensify the odor. While it is not harmful to health, osmidrosis can have a real impact on confidence and daily life.

Who it's for

You may want an evaluation if you notice:

  • Strong underarm odor that returns soon after showering
  • Yellowish stains on the underarms of shirts
  • Odor that persists despite deodorants and antiperspirants
  • Heavy underarm sweating along with odor
  • Wet, sticky earwax and a family history of underarm odor

How we treat it at Egg Clinic

A dermatologist first assesses the severity of odor and sweating, your hygiene routine and your medical history. Other causes of body odor, such as infection or certain medications, are considered before planning treatment.

For mild to moderate cases, options may include antibacterial washes, prescription antiperspirants, and botulinum toxin injections to reduce sweating, which can lessen odor by limiting the moisture bacteria feed on. Reducing underarm hair with laser hair removal may also help. For more severe or persistent osmidrosis, procedures that target or remove apocrine glands may be discussed, including surgical options when appropriate. Your physician will explain the benefits, limits and risks of each approach.

What to expect

At your consultation, we discuss your goals and recommend a stepwise plan. Botox injections take about 15 minutes, with mild pinching and a quick return to daily activities; results typically last several months. Procedures targeting apocrine glands take longer, use local anesthesia, and may involve swelling, bruising, tenderness and one to two weeks of limited arm movement. Temporary numbness, bruising or scarring are possible. Improvement varies, and some approaches may need repeat treatment over time.

Aftercare

  • Keep the underarm area clean and dry
  • Avoid deodorant, shaving and strenuous exercise for about 24 hours after injections
  • After procedures, follow wound care and limit arm movement as instructed
  • Wear loose, breathable cotton clothing
  • Change shirts promptly if they become damp
  • Return for follow-up to review progress and plan any maintenance

Frequently asked questions

Is osmidrosis caused by poor hygiene?

No. It is mainly related to genetics and apocrine gland activity. Good hygiene helps, but it often cannot fully control the odor on its own.

Can Botox help with underarm odor?

By reducing sweat, Botox may help lessen odor for some people, particularly when sweating is a major factor. Results are temporary.

Is there a longer-lasting option?

Some procedures aim for longer-lasting reduction, but no treatment can guarantee results. Your physician will explain realistic outcomes and risks.
