What Is Ultherapy Prime? How Live-Imaging Ultrasound Lifting Works
This is a subtitle for your new post

Quick answer: Ultherapy Prime is a non-surgical skin-lifting treatment that uses micro-focused ultrasound to heat deep layers of the skin, including the SMAS layer that surgeons tighten in a facelift. Its main difference from earlier ultrasound lifting is live imaging: the practitioner sees the tissue layers on screen and places the energy at the right depth. Results build gradually over several months as new collagen forms.
What is Ultherapy Prime?
Ultherapy Prime is the newest version of the Ultherapy platform made by Merz Aesthetics. It delivers focused ultrasound energy to small points under the skin, heating the tissue to roughly 60–70°C. The surface of the skin is left intact, which is why there is no wound to heal.
The heat causes tiny areas of controlled injury. The body responds by contracting existing collagen and building new collagen and elastin. Over the following months the treated skin becomes firmer and slightly lifted.
How does live imaging change the treatment?
Skin and fat thickness vary from person to person, and even between the forehead, cheeks and jawline of the same face. With older "blind" devices, the depth was chosen in advance and applied evenly.
Ultherapy Prime shows an ultrasound image of the layers beneath the transducer in real time. The practitioner can confirm that the transducer is sitting flat on the skin, see where the deeper layers lie and choose the depth for each area. This helps place energy where collagen-rich tissue is, and away from where it is not needed.
How deep does it work?
Three treatment depths are used, each with its own transducer:
- 4.5 mm: the SMAS and fibromuscular layer, the main target for lifting
- 3.0 mm: the deep dermis, for firmness and support
- 1.5 mm: the superficial dermis, for fine lines and skin quality, often around the eyes and on the décolleté
Most face treatments combine two or three depths in one session.
Which areas can it treat?
According to the manufacturer, Ultherapy Prime is FDA-cleared to:
- lift the eyebrow, the neck and the skin under the chin
- improve lines and wrinkles on the décolleté (upper chest)
- improve the appearance of loose skin on the abdomen, the front and back of the arms, and the knees
In practice, it is also widely used along the jawline, the lower face and the cheeks to soften jowls and sharpen facial contour.
What happens during a session?
- Consultation and mapping: the doctor assesses laxity and skin thickness and marks the treatment zones.
- Comfort measures: numbing cream or pain relief may be used, depending on the area and your sensitivity.
- Treatment: the transducer is placed on the skin, the image is checked and lines of energy are delivered zone by zone.
- Afterwards: you can usually return to normal activities the same day.
A full face and neck session commonly takes 60 to 90 minutes. Sensation is usually described as brief, deep heat or prickling with each pulse, strongest over bony areas.
When will I see results, and how long do they last?
Some people notice a slight tightening right away, but the real change comes from collagen remodeling, which is gradual. Improvement typically becomes visible over two to three months and can continue for up to about six months. The manufacturer reports that results can last a year or more after a single session.
Because skin keeps ageing, many people choose a maintenance treatment every 12 to 18 months. Results vary with age, skin quality and how much laxity there is to begin with.
Who is a good candidate?
Ultherapy Prime suits people with mild to moderate skin laxity who want a natural-looking lift without surgery or downtime. It is often chosen by people in their 30s to 50s who notice early jowling, a softer jawline or a lower brow.
It is not a replacement for a surgical facelift when sagging is severe. It is also not suitable over open wounds, active skin infections or certain implants in the treatment area. Your doctor will review your medical history before treatment.
Side effects and downtime
There is no wound and usually no downtime. Common, temporary effects include:
- redness for a few hours
- mild swelling or tenderness for a few days
- occasional small bruises or tingling that settle on their own
Rare effects such as temporary numbness or nerve irritation are possible, which is why depth control and an experienced practitioner matter.
Frequently asked questions
Is Ultherapy Prime the same as HIFU?
Both use focused ultrasound. Ultherapy Prime is a specific branded device with built-in real-time imaging, while "HIFU" is a general term that also covers many other devices without visualization.
Does Ultherapy Prime hurt?
Most people feel short bursts of deep heat. Discomfort depends on the area and the depth used, and numbing or pain relief can make it more comfortable.
Can I combine it with other treatments?
Yes. It is often paired with radiofrequency tightening for skin quality, or with skin boosters and fillers for volume and texture. Your doctor will plan the order and timing.
How many sessions do I need?
One session is usually enough for a visible result, followed by maintenance about once a year or as your doctor recommends.
